the permission form is enabled for fill in and submission via email.  Permission forms require signatures- which may be digital signatures - or the forms may be printed out after typing in the student contact infromation.

First, right click on the permission form link and save it to your desktop.  Then open the form and complete it.  SAve a copy of the form to your computer with all your infomration in it - and then attach it to an email.

PLEASE FILL IN THE FORMS BY TYPING and not printing and handwriting.

You will need an updated version of Adobe Reader.  If you have an older version of Adobe Reader, you will get an error message.
