The permission forms are enabled for fill in on line prior to printing. Permission forms require signatures- which may be digital signatures or handwritten. There are 2 forms - one for your natural parents, and the 2nd for everyone else.

Remember that you have agreed to the Terms and Conditions by signing up.

1. First, right click on the permission form link and save it to your desktop. It is best to open with Adobe Reader - and not in your browser.  

2. Then open the form and complete as much as possible by typing.  DO NOT PRINT A BLANK FORM AND FILL IT OUT BY HAND. You may add handwritten sections but we must be able to read them - so type as much as you can.

3. Save a copy of the form to your computer with all your information in it - and then attach it to an email.

4.   We accept forms copies by fax or email.  Students need to retain a copy ot bring with them.

PLEASE FILL IN THE FORMS BY TYPING and not handwriting. WE MAY REFUSE TO ACCEPT FORMS IF THEY ARE NOT LEGIBLE.

You will need an updated version of Adobe Reader. 

If you have an older version of Adobe Reader, you will get an error message.
